Below are some forms you may be asked to submit. Complete forms carefully and submit them to the financial aid office at your campus.
If a transcript of your tax return is requested, download the IRS Request for Transcript of Tax Return form (PDF). After completing this form mail or fax it to the proper address as listed on the second page. Once you receive the transcript from the IRS, sign and date your signature before submitting it to the financial aid office.
2013-2014 Forms
Special CircumstancesThe Free Application for Federal Student Aid (FAFSA) does not provide a place for students and/or their families to explain special circumstances. A Professional Judgment refers to the school’s authority to administrate and adjust the data elements on the FAFSA and override a student's status in some circumstances. The decision of the financial aid administrator is final. There is no appeal.
