Equipment Check Out Form

Important Equipment Check Out Information for Requestors

• All requests must be submitted by at least a week (5 business days).
• All items must be returned within 2 business days unless stated otherwise.
• Requestors are responsible for providing the necessary supplies for requested equipment. For example, popcorn kernels, butter, salt, bags, etc.
• All items must be returned in the same condition checked out in. Items not cleaned will be returned to the requestor for cleaning.
• Completing the request form does not guarantee we can accommodate your request. You will receive a confirmation within two business days.
