2012-2013 Trevor's Choice Awards Nomination Form

Use the form below to nominate the LSC-Tomball Registered Student Organizations and club members that you feel did an extraordinary job this school year. Our Trevor's Choice Award committee, comprised of students, faculty, and staff, will review the nominees and winners will be announced at the Trevor's Choice Awards Thursday, April 11.

Nominations may be submitted by any individual and the nominator is not required to be a member of a student organization.

DEADLINE: Nomination must be received no later than 4 p.m. on Friday, March 22.
