Forms for College Sponsored Student Travel

Notice: Planning college sponsored student travel can be fairly time consuming. Please allow ample time to complete the process. At least 48 hours prior to departure, copies of the following must be submitted to the Office of Student Activities: (1) A completed and approved Request for Approval of College Sponsored Student Travel, (2) Trip Roster, (3) Participant Release, and (4) Participant's General Information and Authorization for Medical Treatment-Form A-10. The advisor should carry a copy of each student participant's Form A-10, in case there is an emergency. Please guard this information carefully, as it contains students' personal information. Please note that all airline tickets must be purchased through Jennifer Ouzenne (X5448) in the Business Office. If airline tickets are needed, the advisor must submit his/her Request & Authorization for Professional Development and the completed and signed Request for Approval of College Sponsored Student Travel to the Business Office for tickets to be purchased.  Request all forms from your campus Business Office or risk managment representative in Human Resources.

Before the Trip - Advisors must complete the
Request for Approval of College Sponsored Student Travel
Request and Authorization for Leave of Absence  (required for college employees, even if travel is to occur outside of normal working hours)
Trip Roster


Before the Trip - Students complete the following forms:
Participant Release
Participant's General Information and Authorization for Medical Treatment Form (Form A-10  -  must be completed by each student participant)
Sample Statement of Commitment and Behavior (This is not a required form...just one that has been used in the past by the Office of Student Activities. You can copy and paste it, and tailor it to suit your group's needs)

After the Trip - Advisors submit the following (as needed)--Incident Report (Form A-12)


Other Information Regarding Vehicle Usage and Student Travel

Vehicle Use Question and Answer Sheet (not a form, but contains useful information for advisors planning to travel with students)
Request for Information from Texas Driver's Licensing Records (required if an advisor is planning to drive students in a rental vehicle...remember that you should not transport students in your personal vehicle)
